Requesting Records
You have the right to request a copy of your health care records. To request records, please submit a written request to jasmine@theholdingspace.org that includes your full name, date of birth, contact information, and a description of the records being requested. Requests will be processed in accordance with applicable state and federal laws.
Client Rights & Concerns
Your experience in therapy matters. You have the right to have questions or concerns heard and addressed in a timely and respectful manner. I encourage clients to raise concerns directly with me whenever possible so we can work toward resolution together.
If we are unable to resolve a concern to your satisfaction, you have the right to contact the appropriate regulatory or oversight agencies, depending on your state of residence.

Texas Consumer Complaint
If you believe that you have a Consumer Complaint regarding the privacy and security of your health information, you may contact the Texas Office of the Attorney General and file a consumer complaint using this link or by calling the Consumer Protection Division at (800) 621-0508.
